چکیده

Rectal cancer constitutes about one third of all colorectal cancer cases. Total mesorectal excision (TME), first described by Heald, has become the gold standard in rectal cancer treatment, since it was shown to provide a dramatic reduction in local recurrence rates. However, TME does not involve any approaches for lateral pelvic lymph nodes (LPLN), which may be a source of local recurrences. Tumor containing LPLNs were reported to be found in about 10% 20% of the rectal cancer patients. In Western countries, lateral pelvic node involvement is considered as metastatic disease, therefore the standard treatment is total mesorectal excision combined with neoadjuvant chemoradiotherapy. Whereas in Japan, lateral pelvic node metastasis is accepted to be curable with excision, and dissection of lateral pelvic lymph nodes is performed along with total mesorectal excision for the treatment of locally advanced rectal cancers.

Lateral pelvic lymph node dissection after neoadjuvant chemo-radiation for preoperative enlarged lateral nodes in advanced low rectal cancer: study protocol for a randomized controlled trial

BACKGROUND Lateral lymph node (LLN) metastasis is a major cause of local recurrence of advanced rectal cancer. Although there is much controversy between Western and Eastern countries on whether lateral pelvic lymph node dissection (LLND) or neoadjuvant chemo-radiation (nCRT) is preferable for the treatment of LLN metastases, existing retrospective cohorts mainly focus on all middle/low advance...

Analysis of lymph node metastatic pattern according to the depth of in-growth in the muscularis propria in T2 rectal cancer for lateral lymph node dissection.

BACKGROUND/AIMS The biological behavior of rectal cancers that invade the muscularis propria (pT2) has not been well studied. We retrospectively studied the pattern of lymph node metastases in patients with T2 rectal cancer. METHODS We enrolled 88 patients who had undergone curative resection of T2 colorectal cancer through mesorectal excision and lateral pelvic lymph node dissection; we micr...

Where does the first lateral pelvic lymph node receive drainage from?

BACKGROUND Lateral pelvic lymph node dissection (LPLD) in the treatment of rectal cancer has risks and benefits. Avoidance of unnecessary LPLD is important, however, preoperative and/or intraoperative accurate detection of lateral lymph node metastases have not been established. If the lateral lymph node to which the fluid first spread from the primary lower rectal cancer is detected accurately...

Initial Clinical Experience with Robotic Lateral Pelvic Lymph Node Dissection for Advanced Rectal Cancer

PURPOSE This study was conducted to evaluate the technical feasibility and safety of robotic extended lateral pelvic lymph node dissection (LPLD) in patients with advanced low rectal cancer. METHODS A review of a prospectively-collected database at Kyungpook National University Medical Center from January 2011 to November revealed a series of 8 consecutive robotic LPLD cases with a preoperati...

Laparoscopic total pelvic exenteration using transanal minimal invasive surgery technique with en bloc bilateral lymph node dissection for advanced rectal cancer

A 59-year-old man presenting with fecal occult blood visited our hospital. He was diagnosed with advanced lower rectal cancer, which was contiguous with the prostate and the left seminal vesicle. There were no metastatic lesions with lymph nodes or other organs. We performed laparoscopic total pelvic exenteration (LTPE) using transanal minimal invasive surgery technique with bilateral en bloc l...
